You will then be able to print the completed form, to be signed and returned via regular postal mail to the address provided below (it will appear on the next page after clicking CREATE FORM, for your convenience).

Sign-Up Fee is $130.00 and parents purchase equipment direct from Riddell. Logo merchandise can be purchased at www.riddell.com. Mail your completed, signed form with payment to:
